We are a Tier 4 Patient Centered Primary Care Home. We have the intention and commitment to provide an innovative system of medical care.

Our clinic was developed 3 years ago with a forward leadership directive in preventative healthcare measures and reducing medical costs through the Triple Aim.

We created our clinic with the mission to improve population health, enhance patient experience of care and reduce the rate of cost of care.

We have a dedicated CareTeam and Medical Support staff to identify resources that are essential to improve your health and wellbeing. It’s one clinic built as a community to encourage long-term relationships with our patients, and care for you from birth through end of life.

NEW PATIENTS

We are accepting new patients including Oregon Health Plan and Medicare patients. We are currently covered with most private insurers, accepting Motor Vehicle and Workers Compensation Claims as well.

Please arrive 10 minutes prior to your scheduled appointment time. Please bring picture ID and insurance card.

OIH has appointment slots that are reserved for urgent care health concerns. This may include: Urinary tract infections (UTI), Pink eye, Sore Throat, Flu, Common cold, Ear Infection, Strep throat, Head lice, STI check, Sinus Infection
